Requesting Proxy Access for Patients Over 16 years
To request that someone has proxy access to your online medical records, please come into the surgery to fill out a form. You will need to bring ID with you, and the person who you are granting proxy access to will also need to come with you and present ID.
If you are a housebound patient, your nominated proxy can come to the surgery with your signed form and their ID, and the surgery will contact you and verify over the phone that you have consented to proxy access being setup.
Approval might not be granted immediately – some requests may be sent to the GP to approve. This is to ensure that we can appropriately safeguard our vulnerable patients. There is no set time frame for how quickly a request will be dealt with.
Proxy access for Children under 13 years old
The parent or guardian of a child under 13 years old can complete the form and present ID and proof of responsibility (e.g., birth certificate) to the reception.

Proxy access for Children 13 years old and older
The child needs to complete the form and come with the requested proxy to the surgery. Both the child and the proxy will need to present ID and the child will be asked that they have consented to proxy access.
